Why giant cell arteritis cannot be a wait-and-see headache
A new headache in a person over 50 is common in primary care, but giant cell arteritis (GCA) is the diagnosis clinicians cannot afford to miss. This medium- and large-vessel vasculitis can inflame the temporal arteries, the ophthalmic circulation, and the aorta. The clinical problem is not pain alone; it is preventable, sometimes irreversible vision loss that may occur within hours to days when ischemia reaches the optic nerve or retina.
For patients, the challenge is recognizing when a headache is not behaving like an ordinary headache. For clinicians, the challenge is treating before every diagnostic box is checked, while still confirming the disease and limiting glucocorticoid toxicity.
The symptom pattern that should change the tempo
GCA typically affects adults older than 50, with incidence rising into the eighth decade. It overlaps biologically with polymyalgia rheumatica, the inflammatory syndrome of shoulder and hip girdle pain and morning stiffness. The shared mechanism is immune activation within vessel walls, driven by dendritic cells, T cells, macrophages, interleukin-6, and other cytokine pathways that thicken the arterial wall and narrow blood flow.
The classic presentation is a new temporal headache with scalp tenderness. Yet many patients present less neatly: jaw fatigue while chewing, tongue pain, low-grade fever, unexplained anemia, weight loss, or elevated inflammatory markers. Jaw claudication is particularly specific because chewing increases demand in an artery that may already be narrowed.
Vision symptoms are emergency symptoms
Transient vision loss, double vision, new visual field defects, or sudden blurred vision can be warning ischemic events. Amaurosis fugax, a curtain-like temporary loss of vision, should be treated as a threatened stroke of the eye. Once arteritic anterior ischemic optic neuropathy occurs, recovery is often incomplete, and the fellow eye may be at risk without rapid treatment.
How the diagnosis is built, not guessed
The 2022 American College of Rheumatology/EULAR classification criteria assign weighted points for age, inflammatory markers, cranial symptoms, imaging, and biopsy findings. Classification criteria are designed for research cohorts, not bedside diagnosis, but they reflect how modern clinicians weigh probability. A convincing clinical picture plus elevated erythrocyte sedimentation rate or C-reactive protein raises concern; normal markers reduce probability but do not absolutely exclude disease.
Temporal artery ultrasound has become more prominent in EULAR recommendations, especially where experienced operators can identify the “halo sign,” a dark circumferential vessel-wall edema. Temporal artery biopsy remains useful, particularly when ultrasound is unavailable or negative despite strong suspicion. CT angiography, MR angiography, or PET-CT may identify large-vessel involvement affecting the aorta and its branches.
A common misconception
One misconception is that GCA is excluded if the temporal artery is not tender or the headache is not exactly at the temple. In reality, inflammation can be segmental and systemic. Another misconception is that clinicians should wait for biopsy before prescribing steroids. When vision is threatened, treatment is started because the consequence of delay is greater than the downside of briefly treating a suspected case.
When it becomes an emergency
Consider a 73-year-old patient with five days of new scalp tenderness and fatigue who wakes with brief darkness in one eye while making coffee. The headache matters, but the transient monocular vision loss changes the risk category. The working question is no longer, “Could this be GCA?” It becomes, “How quickly can we protect vision while confirming the diagnosis?”
In that setting, emergency evaluation generally includes urgent glucocorticoids, ophthalmologic assessment, inflammatory markers, blood counts, liver tests, and vascular imaging or biopsy planning. Intravenous methylprednisolone is often used when there is acute visual loss or amaurosis fugax; high-dose oral prednisone is commonly used for cranial GCA without established vision loss. The exact regimen depends on severity, comorbidities, and specialist input.
Red flags that should accelerate care
- Sudden, transient, or persistent vision loss
- Double vision or new eye-movement symptoms
- Jaw claudication, tongue pain, or scalp tenderness with new headache
- Stroke-like symptoms, limb claudication, or unexplained systemic inflammation
Why treatment is more than a steroid taper
Glucocorticoids rapidly suppress vascular inflammation, but prolonged exposure causes predictable harm: infection, osteoporosis, diabetes worsening, hypertension, cataracts, mood changes, and muscle loss. The clinical art is to prevent ischemic events first, then reduce cumulative steroid burden without allowing relapse.
The 2021 ACR/Vasculitis Foundation guideline conditionally recommends adding tocilizumab, an interleukin-6 receptor inhibitor, with glucocorticoids for newly diagnosed GCA in many patients. The GiACTA trial showed that weekly or every-other-week tocilizumab with a 26-week prednisone taper achieved higher sustained remission at one year than prednisone taper alone. This does not mean every patient needs biologic therapy; it means steroid-sparing treatment should be considered early, especially with relapsing disease, high steroid risk, or large-vessel involvement.
Methotrexate has modest evidence as an alternative steroid-sparing agent when tocilizumab is not appropriate. Aspirin is not automatically indicated for everyone; its use depends on ischemic risk and bleeding risk.
Where the evidence stands
GCA is an emergency when vascular inflammation threatens sight or brain circulation, and it is a chronic vasculitis when relapse prevention and steroid toxicity become the central problems. Current evidence supports rapid glucocorticoid treatment for suspected vision-threatening disease, ultrasound or biopsy to strengthen diagnostic certainty, large-vessel imaging when features suggest extracranial involvement, and early consideration of tocilizumab for steroid sparing.
Uncertainty remains around optimal taper speed, best monitoring strategies when interleukin-6 blockade suppresses C-reactive protein, and which patients can safely avoid biologic therapy. The synthesis is simple: new headache after 50 deserves context; headache plus visual symptoms deserves urgency; confirmed GCA deserves a plan that protects vision today and vascular health.
Steroid-sparing treatment: tocilizumab
The GiACTA trial, published in the New England Journal of Medicine in 2017, showed that weekly or every-other-week tocilizumab plus a 26-week prednisone taper achieved sustained remission more often than prednisone taper alone. Tocilizumab blocks the interleukin-6 receptor, a pathway central to the acute-phase response. The ACR/VF guideline conditionally recommends tocilizumab with glucocorticoids for many newly diagnosed patients, especially those at high risk for steroid toxicity or relapse.
The tradeoff is monitoring. Because IL-6 blockade lowers CRP and fever responses, relapse may be clinically subtle. Clinicians must listen carefully for recurrent cranial symptoms and monitor vascular disease when indicated, rather than relying only on a normal CRP.
A note on normal labs and negative biopsy
Normal inflammatory markers lower probability, but they do not eliminate it. Another misconception is that GCA is simply a headache disorder. It is a systemic vasculitis with cranial and large-vessel consequences. Conversely, not every headache in an older adult is GCA; the clinical pattern, examination, labs, and imaging must cohere.
